Conditions We Treat

Aneurysms

What Is an Aneurysm?

An aneurysm is a swelling or bulge in the wall of a blood vessel caused by a weakness in the vessel wall. Over time, the pressure of blood flowing through the vessel can cause the bulge to expand, increasing the risk of rupture. Aneurysms can occur in any blood vessel but are most common in the aorta, the body’s largest artery, which carries blood from the heart to the rest of the body.

A ruptured aneurysm is a life-threatening emergency requiring immediate medical attention, making early detection and management crucial.

Types of Aneurysms

  • Abdominal Aortic Aneurysm (AAA): A bulge in the part of the aorta that passes through the abdomen.
  • Thoracic Aortic Aneurysm (TAA): A bulge in the section of the aorta located in the chest.
  • Peripheral Aneurysms: These occur in other arteries, such as those in the legs or arms.

Symptoms of an Aneurysm

Many aneurysms develop slowly and do not cause symptoms until they are large or rupture. However, symptoms may include:

  • A pulsating feeling in the abdomen (for abdominal aneurysms).
  • Sudden, severe, or persistent pain in the chest, back, or abdomen.
  • Shortness of breath or difficulty swallowing (for thoracic aneurysms).
  • Pain or swelling in the legs (for peripheral aneurysms).

A ruptured aneurysm can cause severe symptoms such as sudden, intense pain, dizziness, fainting, or shock. If you experience these symptoms, call 999 or go to A&E immediately.

Who Is at Risk?

Factors that increase the risk of developing an aneurysm include:

Smoking: A major risk factor for aneurysms.

 • High Blood Pressure: Puts extra stress on blood vessel walls.

 • Family History: Genetics can play a significant role.

 • Age and Gender: Most common in men over 65.

• Atherosclerosis:  Build-up of fatty deposits in the arteries.

 • Certain Medical Conditions: Such as connective tissue disorders.

Diagnosis of Aneurysms

Aneurysms are often detected during routine screening or investigations for other conditions. Diagnostic tests include:

Ultrasound Scans: Commonly used to detect abdominal aortic aneurysms (AAA).

CT or MRI Scans: Provide detailed images of the aneurysm’s size and location.

Screening Programmes: In the UK, men aged 65 are routinely offered an ultrasound to screen for AAA as part of the NHS Abdominal Aortic Aneurysm Screening Programme.

Treatment Options for Aneurysms

Treatment depends on the size, location, and risk of rupture. At The Vein Surgeons, we follow NICE guidelines and offer tailored treatment plans to ensure the best outcomes.

1. Monitoring and Lifestyle Changes:

•Small aneurysms that are not at immediate risk of rupture may be monitored with regular ultrasound or CT scans.

•Lifestyle changes, such as quitting smoking, managing blood pressure, and maintaining a healthy weight, can help reduce the risk of aneurysm growth.

2. Endovascular Aneurysm Repair (EVAR):

•  EVAR is a minimally invasive procedure where a stent graft is inserted through a small incision in the groin and guided to the aneurysm using imaging.

•  The stent graft reinforces the weakened artery and prevents blood from entering the aneurysm, reducing the risk of rupture.

•  EVAR offers quicker recovery times and less discomfort compared to open surgery.

3. Open Aneurysm Repair:

•  For larger or more complex aneurysms, open surgery may be required.

•  This involves removing the damaged section of the artery and replacing it with a synthetic graft.

•  Although this procedure requires a longer recovery period, it is a proven and durable solution for treating aneurysms.

Aftercare and Recovery

Following treatment, patients are encouraged to adopt healthy lifestyle habits to reduce the risk of complications or future aneurysms. This includes:

• Quitting smoking.

• Managing blood pressure and cholesterol levels.

• Attending regular follow-up appointments to monitor the success of the treatment.
Most patients undergoing EVAR can return to normal activities within two weeks, while those undergoing open repair may require six to twelve weeks for full recovery.
